The Transitions Pathway in Enfield is seeking an experienced & enthusiastic Band 4 mental health engagement worker, dedicated to making a difference to young people in the community.
Confident and dynamic, they might be experienced in support work within CAMHS, be a newly graduated Associate Mental Health Worker or have other relevant experience working in the mental health field.
The candidate should have drive and a sound knowledge of mental health to work in a creative and personalised way.

The Transitions Pathway is part of Enfield Core Community Mental Health Team. The Pathway works closely with CAMHS and core locality services for adult mental health and charity partners, MIND. The Pathway is also linked with other "Front Door" mental health services, including the Community Engagement Team.
Overall, our mission is to ensure safe, supportive and age-appropriate transition from CAMHS to adult services for the young adults under our care.
The successful candidate will work closely with team leads, Experts by Experience, patients, carers and other professionals in the network to establish and refine this new service.
Interest in /experience of working in adolescent and young adult mental health care is vital for this role. As is a flexible and innovative approach to working. The Engagement Worker will help to deliver a goals based approach (with individual and group work), ensuring that young adults’ needs are met at home and the community.
They will assist in the delivery of high standards of care to individual young people. There will be an expectation that the applicant will work independently but will have the support, guidance and direction of a qualified nurse, occupational therapist or social workers and feed into the wider care plan being delivered by the MDT.
Other duties include:
1. Use of Dialog+ and other tools to assess service users health and social care needs
2. Contribution to the design of care plans in line with risk assessment and psychosocial needs.
3. Utilisation of designated recovery tool with outcomes focusing on care planning, ensuring clients understand they will work in a collaborative manner to engage in social inclusion activities such as seeking employment, education and independent living.

In order to meet the needs of the Trust you may be required from time to time to work at different locations to your normal place of work. This may mean that you are required to work at any location that fall under Barnet, Camden, Enfield, Haringey, or Islington.
This is an exciting opportunity to work within a newly created pathway aimed at supporting healthy transition from CAMHS to adult services. The 18-25 Transitions Pathway supports young people between 17-25 who require additional input to ensure they are safely supported into adulthood.
