In this position you will carry out complex health assessments and identify health and social care needs to patients at the Julian House Homeless Centre. The post holder will be responsible for providing health assessment and nursing care for homeless people who have long term health issues who are not regularly accessing primary care services. The role includes ensuring patients are registered with primary care services and are supported to attend appointments to meet their health needs and access preventative care. Main responsibilities: Provision of appropriate and regular screening assessments based on current research in relation to the health needs and problems of homeless people. Management of chronic diseases Health care triage, assessment and treatment Patient advocacy, referral/linkage to primary care, support for medication adherence Health education, support for chronic health conditions, and self-management goal setting and support Deliver proactive promotion of health services to the local homelessness community ensuring that they are aware of the services available to them
