Detailed job description and main responsibilities The role of Support Worker will apply a strength-based approach to work collaboratively with services users and the MDT to facilitate the following: Support in partnership with clinical staff and service users in the planning and implementation of care. Monitoring the services user's progress, level of functioning, mental state and risk, escalating as required to clinical staff. To help guide the process of recovery. To initial recovery and solution focus, through psychological and pharmacological interventions. To liaise and co-ordinate with other professionals. To enable access to community resources. To support service users holistically to include health promotion and social support To undertake any other duties which may be reasonably required within the scope of the post To initiate, organise and evaluate therapeutic support with clinical advice as appropriate. To maintain accurate and timely information, documentation and reporting systems in accordance with agreed standards. To report regularly to the shift co-ordinator and participate in handovers and clinical reviews as appropriate. To preserve the dignity and self-respect of service users at all times. To promote user involvement in all aspects of their care including effective engagement in agreed care plans and supporting them to access identified services on a regular and consistent basis within a team approach. To ensure that professional boundaries are always adhered to. The role requires effective communication and leadership skills, autonomous decision making and dynamic risk assessment skills. We are looking for someone who is adaptable with a positive attitude, that has experience in working in a team whilst having the ability to be proactive and work independently. You will be an active member of the MDT working to ensure that the UCAT service is provided to the highest possible standard, working flexibly to respond to emerging needs and effectively address the needs of the service users. You will be available to work shifts over 7 days a week, 365 days a year.
