An exciting opportunity is available for a Qualified Social Worker to join the Adults Integrated Discharge Hospital Team in Rotherham. This ongoing role offers a competitive hourly rate of £33.00 and the flexibility to work full-time or part-time. Reporting to the Team Manager, you will play a vital role in supporting vulnerable adults through the hospital discharge process, ensuring high-quality care and a collaborative, multi-agency approach.

Key Responsibilities:
1. Deliver personalised social care services for vulnerable adults, supporting statutory social work functions.
2. Conduct assessments and investigations, producing evidence-based reports to inform care planning.
3. Develop and implement support plans that meet individual needs, collaborating with other agencies as required.
4. Monitor and review care plans to ensure high standards of service delivery and adjust them when necessary.
5. Empower clients by promoting self-advocacy and ensuring their voice remains central to service provision.
6. Provide information, advice, and support to clients, carers, and families throughout the discharge process.
7. Work in partnership with multi-agency teams to ensure a seamless and coordinated discharge.
8. Undertake safeguarding investigations and ensure appropriate interventions when required.
9. Contribute to the development of preventative strategies to reduce hospital readmissions and promote independence.
Requirements:
1. Qualifications: Degree or equivalent qualification in Social Work.
2. Registration: Current registration with Social Work England (SWE).
3. Experience: Demonstrable experience working within hospital discharge or adult social care settings.
4. Skills: Strong assessment, communication, and organisational abilities with a commitment to person-centred practice.
