We’re on the hunt for a Care Coordinator to join us in our DCA Office in Eastleigh. This service is a welcoming and friendly environment with a dynamic team that supports a fantastic group of people!
As a Care Coordinator, you’ll play a pivotal role in ensuring the people we support are kept safe by effectively deploying our care delivery colleagues and ensuring all administration is undertaken in line with relevant regulations and legislation. You’ll manage and coordinate the care delivery team, ensuring that the relevant documentation is in place for new people we support, discharges, and transfers. You’ll conduct live call monitoring using bespoke software and be the first point of contact for day-to-day operational requirements of our care colleagues. You will also work alongside the management team to coordinate with families, healthcare professionals, and external agencies, handle enquiries, schedule appointments and meetings, and manage all information with confidentiality.

Our Care Coordinators should have experience working in Health & Social Care, proficiency in Microsoft Office, prior experience in an administration role, and experience providing excellent customer service.
