East and Central Brighton PCN is seeking a dedicated Healthcare Assistant (HCA) to join our team as part of a cardiovascular project. The project is focused on promoting cardiovascular health within our community and providing essential monitoring and support for patients. This role is pivotal in delivering direct patient care, including phlebotomy, blood pressure monitoring, and pulse checks, and in educating patients on lifestyle factors related to cardiovascular health. Key Responsibilities: Phlebotomy: Conduct blood draws efficiently and safely, ensuring high standards of patient comfort and care. Vital Sign Monitoring: Perform regular blood pressure and pulse checks, record and report findings accurately, and promptly escalate any abnormalities to the relevant clinician. Health Promotion: Educate patients on lifestyle changes and preventative measures associated with cardiovascular disease, including diet, exercise, and smoking cessation. Provide accessible and understandable information to empower patients in managing their health. Patient Support: Offer empathetic support to patients, addressing their questions and concerns about cardiovascular care and promoting a welcoming and reassuring environment. Record Keeping: Maintain accurate and timely patient records in line with PCN standards and confidentiality guidelines. Team Collaboration: Work closely with other members of the PCN, including GPs, nurses, social prescribers, and pharmacists, to deliver comprehensive and coordinated care. Future Opportunities:Upon the conclusion of this cardiovascular project, there will be opportunities to continue developing and applying your skills within the PCN. Possible areas of involvement include (but are not limited to): Long-term condition reviews, such as asthma and diabetes checks Health checks and screenings for patients with learning disabilities Frailty support and community outreach initiatives
