The prison houses around 640 patients where we deliver the primary care, mental health and substance misuse services. The site currently offers shifts across Monday to Sunday including bank holidays. In this role you will work 7 nights on and 7 nights off, from 20:30 - 07:45. This position is full time, youll receive an annual salary up to £38,115 per annum - depending on experience. Main duties of the job
As a Primary Care Practitioner, your responsibilities will vary. You will:
1. Work within a multi-disciplinary team to deliver a nurse led service and prevent hospital admissions
2. Undertake medication rounds, dispense controlled medication, attend emergency response situations, assess patients, and undertake reception screening
3. Use your skills and knowledge to deliver nurse led clinics to your patient group whilst being supported by a multi-disciplinary team.

Qualifications required
11. Registered nurse Adult / Paramedic
12. Current professional qualification (NMC)
13. Evidence of Continued Professional Development
14. Understanding of the complexities of multi-disciplinary and multi-agency working
15. A non-judgmental and compassionate approach
16. Excellent communication skills
17. Understanding of clinical governance, health and safety, QOF, EBP and national standards
18. Knowledge of healthcare policies and best practice

Job responsibilities
To provide healthcare to patientson site. This will include some emergency care such as suturing, providingemergency response to undertaking dressings and performing a primary carefunction. There is a focus on health promotion and keeping the patients withinthe prison, safe and well by providing effective safe care. You will lead ateam of healthcare practitioners some in specialist roles to deliver aneffective safe medical service within the prison. As the Primary Care Practitioner (Adult Nurse or Paramedic) yourresponsibilities will include ensuring patients have medical plans of care,assessing patients, carrying out relevant clinical investigations, and arrangefollow-up appointments whilst offering advice and giving information. Must beable to clinically evaluate and ensure the safe transfer of patients to acutehospitals, as well as ensuring that plans of care are understood by the patientand the prison as appropriate. You must be able to demonstrate criticalthinking skills in clinical decision making. As part of your role you willsupport in undertaking audits and governance.
