To be able to work both autonomously and within a team, to support our patients on their TB journey. The post holder is a key member of the TB team whose overarching goal is to deliver quality clinical and nursing care and TB prevention and control.
The post is part time (22.5 hours weekly 9 to 5) excluding Bank Holidays and weekends, with a base in Gt Yarmouth. The post-holder will work as part of the Community TB nursing team, providing community care across the Gt Yarmouth and Waveney footprint. Main duties of the job
The TB Outreach Nurse will be a member of the TB service providing support to patients and their families delivering expert patient-centred nursing care to patients being treated for Tuberculosis (TB) and Latent TB Infection (LTBI). The role is patient facing and will include supporting nurse led clinics, home visits, community drop in/outreach work, and facilitating directly observed therapy. You will be working closely with other agencies, providing education, and raising awareness of TB. You will help to provide a flexible and responsive high quality TB service facilitating effective patient management and protection to the general public.
Job responsibilities
1. To deliver nurse led Screening clinics as directed by senior colleagues.
2. To manage a delegated caseload of patients.
3. To provide an outreach service by visiting patients at home, in hostels and a variety of temporary accommodation settings.
4. To ensure, at all times, that the needs of the patient are placed at the centre of service delivery.
5. Recognises, investigate and plans in consultation with the TB Nurse Specialist, the necessary action to resolve problems in relation to patient with adherence problem or likely to default treatment.
6. Providing support to patients facilitating regular clinic follow up and assist in locating patients at risk of defaulting on treatment.
7. Develop links and formalised referral methods with relevant services and to establish and maintain collaborative care networks.
8. To undertake audit of relevant TB Service activities to ensure that standards for TB Infection prevention and control are met.
9. To investigate contacts of TB index cases by delivering TB Contact Tracing in domiciliary and outpatient clinic settings. Collecting laboratory data and clinical information.
10. Deputise in the absence of line manager.
11. To risk assess and develop individual care plans in partnership with clients based on a shared assessment of need.
12. To formulate and review/update TB service Policies as directed.
13. Attend in service training courses and study sessions as appropriate to maintain own competencies.
14. Plan, implement, deliver and evaluate TB training package to students and other members of ECCH staff.
15. To act as a source of expert advice regarding TB infection information to patients, contacts and members of the public.
16. Maintain contemporaneous and accurate written and electronic records in partnership with clients and in accordance with policies and procedures.
17. Provide day to day supervision to others in line managers absence.
18. To lead on tasks as delegated by line manager.
19. Write reports and participate in audit on service delivery, as required for manager.

Experience
* Effective caseload management.

Qualifications
* RGN currently registered with the NMC.
* Educated to degree or equivalent experience.
* Requirement to undertake TB Diploma via distance learning.
* ENB 998 or equivalent.
Skills and Knowledge

* Phlebotomy skills and experience.
