We welcome applications from Occupational Therapists with a keen interest in working within an integrated community team, with proven clinical expert experience in neuro rehabilitation. The successful applicant will need to show a driven, positive, self motivated approach to work with the team to further develop and enhance the service provision for community neuro patients across East Cheshire. This post is based across the East Cheshire locality, with a flexible base.It is therefore necessary to travel efficiently between sites and to rural areas. Hours per week: 18 hours per week Main duties of the job Provide clinical expertise, making decisions about the best course of action for patients' care. This will involve assessing patients and providing personalised rehabilitation plans, whilst working alongside multiple agencies, to determine the best management plan for patients. To provide leadership and clinical supervision to junior staff members Deliver programmes of supported patient rehabilitation, in ways that facilitate behavioral change, optimise individuals' physical activity, mobility, fulfilment of personal goals and independence, and that ensure patients can be maintained safely within their own homes, if possible.
