Job summary The overall aims of the Adult Community Healthcare Team are to avoid inappropriate admission into the acute service, facilitate timely discharge into the community and enable individuals to be cared in their home environment. The ACHT reflects an integrated approach that has been designed in response to the changing needs of the local population, with a special emphasis being placed on the proactive vision of care for patients in the community setting. The post will involve working in the community, supporting the community nurses and promoting independence in the patient's home environment.This is an exciting and innovative service. Candidates to be experienced carers and be able to demonstrate having experience or knowledge of supporting patients in their own homes. You will have excellent communication skills and the ability to work with a range of health professionals and informal carers. The ideal candidate will be flexible and adaptable, being able to respond to need at short notice.In order to respond to the healthcare needs of the local population, the services covers a 7 days a week rota which therefore includes weekends and bank holidaysThe candidate must hold or work towards the NVQ level 3 in Health and Social Care or equivalent. The post holder will be appropriately trained and supervised in various skills to work independently supporting the District nursing service. Please Note: Being a car driver and owner is essential. Main duties of the job To work alone using initiative and a wide range of health and social care skills, within the guidance set and agreed by the professional members of the team. The term 'professional' member of the team refers to those clinicians, social workers etc within the team who make the initial assessment and are responsible for the patient's case management. To prepare for and provide care and interventions enabling patients in their own home or community based facility. You will follow the care plan written by the patient's case manager. To take part in the Adult Community Healthcare Team's experiential training scheme To undertake training to acquire the skills appropriate for development in the post To deliver care in a way that understands the differences in values and beliefs held by the patient and supports their right to privacy, dignity, preferences and choices. To obtain and understand the importance of consent for the patient in all interventions. To develop relationships with patients and carers who value them as individuals, communicating effectively to promote the patients goals.
