Banbury Alliance PCN are looking to recruit a Social Prescribing Link Worker.
We are looking for someone to work closely with our care coordinator and other team members across our PCN to improve health inequalities and reduce deprivation across our PCN, which consists of 3 GP practices and a total population of 30,000.
Social prescribing empowers people to take control of their health and wellbeing through referral to link workers who give time, focus on what matters, and take a holistic approach to an individual’s health and wellbeing, connecting people to community groups and statutory services for practical and emotional support.
Link workers also support existing groups to be accessible and sustainable and help people to start new community groups, working collaboratively with all local partners. It is particularly beneficial for people with long-term conditions, for people who are lonely or isolated, or frail, or have complex social needs which affect all aspects of their life.
We encourage you to apply if you have an interest in helping to tackle health inequalities and have knowledge of frailty care. Knowledge of Banbury is preferred due to the nature of the role.

* Willingness to undertake training and develop skills
* Enrolled in, undertaking, or qualified in level three occupational standard (PCI Social prescribing)
