The successful applicant will be an NHS employee and may therefore have contact with vulnerable service users. Responsibilities
1. To assist the Ofsted Registered Manager in partnership with both family and other professionals in providing the appropriate care package, ensuring continuity and a consistent approach.
2. To ensure the provision of a warm and homely environment that meets the emotional, physical, cultural, religious, and spiritual needs of the child/young adult.
3. To provide training and support to develop staff knowledge and skills that reflect the needs of each individual child and young person.
4. Hours of work will be flexible to meet the needs of the service. Mostly 12-hour shifts days, nights, weekends. The home is at present commissioned to open for five nights a week.
5. To work in partnership with the Multi-Disciplinary Community Team for Children and Young People with Learning Disabilities, maintaining responsibility for a designated caseload of children who attend Roston supporting healthcare support workers identified as children's link support worker at the Home.

Management and Personal Development
1. To have responsibility for leading a shift, working alongside skilled support staff, providing a residential short break to the children and young people who attend Roston and on occasion Layland.
2. To work under the Children's homes regulations including the Quality standards (Ofsted).
3. To participate in Multi-disciplinary Team meetings and complex case discussions where appropriate.
4. To complete training on all medical/clinical procedures required for the home.
5. To ensure effective management of allocated caseload at the residential Home and submit written reports and statistical returns as required.
6. In liaison with the Ofsted manager ensure that clinical advice and guidance to staff is given so that all medical, behavioural, and risk-taking issues for children are adequately addressed and monitored closely, and that professional standards of conduct and activity are maintained.
7. In liaison with the Ofsted registered manager ensure all staff within the Home have opportunity for annual appraisal, development of practice.
8. To liaise effectively with other healthcare professionals to ensure that work is neither overlooked nor duplicated, building and sustaining effective communications with other roles involved in the local health community as required to ensure good team working and collaborative working practices are delivered to children with learning disabilities.
9. To assist in providing clinical supervision for support staff working within the Home.
10. To supervise and act as mentor for the teaching and placement of learners and healthcare support staff.
11. To exercise awareness of professional responsibility by making time to study, taking advantage of opportunities for continuing professional development in line with PREP and the development of the role, whilst reading current relative literature and having an up-to-date knowledge of learning disability nursing issues.
12. Actively participate and engage in own professional, safeguarding supervision and line management support.
13. To be familiar with and adhere to current Trust policies and procedures.
14. To demonstrate anti-discriminatory practice in all aspects of work.
15. Organise and manage own time and caseload, prioritising as necessary according to workload.
16. To adhere to the Nursing and Midwifery Council's Code of Professional Conduct ensuring NMC registration is maintained.
Clinical Activities
1. To provide an environment where a child can enjoy and participate in choosing appropriate activities.
2. To encourage a child to develop new skills and experiences, in a positive and caring way.
3. To provide a service in which the whole family and child can have involvement in their care package.
4. To provide advice, consultation, support, and information to others, to ensure that the health needs of individual children and young people with learning disability are met appropriately.
5. To value the diversity of each and every child, acknowledging that each child and family have different needs.
6. To respect the right of each individual child/young adult to privacy and dignity.
7. To value and be supportive to every child/young adult and family, in ensuring they receive a high-quality service.
8. To provide direct care to the children and young people; this may constitute assistance with personal care, medication administration, light household tasks, and meal preparation.
9. To undertake nursing assessments of children, young people, and families with significant mental health problems associated with learning disability, including challenging behaviours, autistic spectrum disorder, social, communication, sexuality, and mental health needs in relation to their residential short break placement.
10. To complete specialist nursing assessments, develop bespoke care plans, and implement interventions to promote the health and wellbeing of children and young people including positive behaviour support plans.
11. To positively represent the needs and rights of children and young people with learning disabilities, promoting equality, and assisting access to NHS services.
12. To make clinical judgements and document and act on any significant changes in any child's personal circumstances, liaising with other professionals as necessary to meet the changed needs.
13. To enter clinical information, in a timely way, according to Trust policy.
14. To administer medication following trust policy and procedures.
15. To work as a knowledgeable practitioner within the wider team.
16. To work collaboratively with colleagues to ensure the smooth transition of care between services.
17. To develop positive and professional interpersonal relationships with children, their families, and colleagues, maintaining a professional attitude at all times.
18. To work as a member of the multi-disciplinary team, maintaining excellent working relationships with other professionals, voluntary workers, and significant others.
19. To perform all functions of the post consistent with legislation, such as The Children Act 1989/2004; Mental Health Act 1983/2007 and child safeguarding procedures.
20. To participate in child protection procedures as required, taking appropriate action where necessary in relation to the protection of safeguarding children/young adults.
21. To communicate complex information, which may be challenged or contested by the receiver, because of an alternative viewpoint, the presence of psychiatric disturbance, challenging behaviour, communication difficulty, or impaired insight.
22. At all times promoting inclusion for children with learning disabilities in all health promotion initiatives across Cornwall health services.
