Job summary
We are seeking a motivated Occupational Therapist to join our Locality Therapy Team. The successful individual will be based at Ludlow Hospital, working on the Community Hospital ward and as part of the Community Team visiting patients in their own homes. They will join an enthusiastic and supportive multi-disciplinary team in providing high quality care, rehabilitation, and discharge planning.
The successful individual must have an interest and experience in patient rehabilitation within the community setting and be able to work as an autonomous practitioner. The main responsibilities include providing Occupational Therapy assessments and rehabilitation to patients with a wide range of conditions and complex needs.
We offer a strong commitment to clinical supervision and Continuing Professional Development (CPD), providing a supportive environment with opportunities for supervision, training, and peer support both locally and across the county. The post will involve supervising junior staff, rehabilitation technicians, and students.

Main duties of the job
1. To deliver a high-quality, seamless Occupational Therapy service from acute or community hospitals to the community, responsive to the needs of the local population, as part of a multidisciplinary team.
2. To work as a key member of the multidisciplinary team, responding flexibly to changing patient needs.
3. To facilitate seamless transition from community hospital to the patient's own home and transfer care to appropriate services to maximize independence and quality of life.
4. To provide supervision and support the personal and professional development of junior staff, enhancing their skills and knowledge, including line management where appropriate.
