Apply now to embark on this transformative journey with us Main duties of the job Key Responsibilities • Lead, develop, and deliver patient-centred pharmacy services across cardiology alongside utilising prescribing skill set. • Collaborate with clinical colleagues in a fully integrated MDT to meet the pharmaceutical needs of the cardiac unit. • Participate in advanced practice opportunities and contribute to the ongoing development of cardiology services. • Create and present regular reports to the NICE high cost group with outcomes and improvements The role will involve working collaboratively with pharmacy colleagues, clinicians, nursing staff and other members of the multi-disciplinary team to ensure the delivery of high-quality pharmaceutical care to cardiac patients. The role will be based within cardiologys MDT with a focus on the weekly ward take and facilitating homecare prescriptions. The role will also involve identifying areas for improvement in pharmaceutical services within the cardiac unit and implement strategies to enhance efficiency, quality, and patient satisfaction. Supervision, mentoring and training of junior colleagues will also be a major component of the job. Delivering an independent prescribing service and managing pharmacy aspects of the Cardiac Home Delivery Service. Job responsibilities The ability to speak Welsh is desirable for this post. English and/or Welsh speakers are equally welcome to apply. The post holder will participate in dispensary duties and provide clinical leadership to junior pharmacists and trainees. In addition, the role will require the ongoing completion of core directorate activities including analysis of medicines expenditure, clinical governance audits, service improvement projects and risk management. Additionally, you will be required to participate in weekend, Bank Holiday and out of hours rotas as dictated by the needs of the service. Person Specification Qualifications Essential • Post Graduate MSc/ Diploma in Clinical Pharmacy or equivalent experience • Member of the GPhC Desirable • Non medical prescriber • Accredited Tutor for clinical pharmacy diploma Experience Essential • Ability to undertake autonomous practice. • Experience as clinical pharmacist in specialist area. • Evidence of clinical audit, practise research, implementation of procedures, protocols. Desirable • Lecture on specialist subject. • Experience of being a pre-registration pharmacist tutor.
