Merton Domiciliary Therapies and Merton Holistic Assessment and Rapid Intervention Team is a Multidisciplinary Team providing high-quality rehabilitation to the Merton Population.
The post holder will support adults living with frailty and complex health needs to optimise their independence, health, and wellbeing, and prevent admission into hospital or premature move to a care home. We are strongly committed to the assessment and rehabilitation of patients who are living within the Merton community.
The post holder has a key role within the team working with patients, service users, and their carers in providing highly specialist occupational therapy (OT) assessment and treatment plans to assist individuals to maintain or regain their mobility, independence, and wellbeing within their own homes, setting goals and reviewing outcomes in order to reduce the need for long-term support.
The post holder will provide local clinical leadership and advice for the OTs and MDT in domiciliary therapies and be responsible for the education and supervision of junior occupational therapy staff, rehabilitation assistants, and OT students.

Main duties of the job
1. Providing a highly specialist occupational therapy (OT) service as part of a multi-disciplinary team, to patients in their own homes and within a clinic setting.
2. To provide clinically effective, person-centred, and evidence-based occupational therapy assessment and intervention to patients with complex physical health needs.
3. To assist the Clinical Lead in the planning and delivery of occupational therapy services to the patients, their family/carers, students, and apprentices complying with RCOT professional and service standards of practice.
4. To provide support and supervision for junior staff.
5. To assist in the management, development, and research of occupational therapy services, including service reviews. To hold responsibility for defined projects.
6. To provide support and training to all staff with regard to occupational therapy interventions.
7. To comply with the College of Occupational Therapists Code of Ethics and Professional Conduct.
8. To deputise and lead the service as and when needed in absence of Clinical Lead.
9. To jointly lead clinical triage with other senior therapists and clinical lead in the team.
10. To be responsible for Standard Operating processes in relation to policies and processes that are needed for smooth functioning of the team.
11. To work flexibly and travel to multiple sites as required.

* Educated to degree level with appropriate post-graduate qualification in relevant clinical area.
* HCPC registration.
* Evidence of CPD relevant to the treatment of people with physical disabilities.
* Up to date CPD portfolio.

* MSC in appropriate clinical area.

* Post-registration experience at band 6 level or above in a wide range of clinical areas i.e. elderly rehabilitation, community rehabilitation, and orthopaedics.

* Postgraduate experience of working in a multi-professional team.
* Experience in teaching and training and facilitating groups therapy sessions.
