Job Overview
Marlow Adult Community Health Care Team (ACHT) is delighted to announce a Band 6 District Nurse/Community Nurse/Case Manager position:
* 1 x full time 37.5 hours per week
The ACHT reflects an integrated approach that has been designed in response to the changing needs of the local population, with a special emphasis on proactive care for patients in the community setting, including caseload management, prevention of admission, reablement, and end-of-life care.
Main Duties of the Job
* Provide interventions tailored to meet the needs of the individual
* Contribute to processes for proactively case finding patients who are high intensity users of community and primary healthcare
* Assess the physical and psycho-social needs of the defined caseload
* Improve health outcomes using specialist nursing skills and knowledge
* Manage a caseload of patients with long-term conditions who would benefit from case management
Person Specification
Education, Qualifications & Training
* Registered Allied Healthcare Professional / Registered Level 1 Nurse
* Degree/Diploma in Health/Nursing Studies or related subject
* Evidence of recent Continuous Professional Development
Skills, Abilities & Knowledge
* Demonstrate specialist clinical assessment and evaluation skills
* Excellent written and verbal communication skills
* Ability to manage complex cases
* Knowledge of legal requirements for working with vulnerable adults
Experience
* Demonstrable experience of working in a community/in-patient setting for a minimum of 3 years post registration
* Experience of working within multi-agency/multi-disciplinary teams
Special Circumstances
* Valid and current driving license or able to travel to locations throughout Buckinghamshire
