Older Adult Home Treatment Team (OAHTT)
Hours: 37.5 per week

Salary: Band 5 £28,407 - £34,581 pa (dependant on experience)
We are looking for enthusiastic and motivated registered mental health nurses to join our Older Adult Home Treatment Team.
The Older Adult Home Treatment Team provides a service 8-8pm, 365 days a year to older adults experiencing a crisis in the context of functional mental health or dementia. We work intensively with service users in a community setting to prevent admission to a mental health ward, support discharge from a mental health ward, and provide comprehensive assessment and treatment plans in the person's own home.
You should be able to demonstrate your passion for working with this client group and have the ability to assess, co-ordinate treatment, and support clients and carers. You should possess effective communication skills and have the ability to work under your own initiative and as part of a wider multidisciplinary team.
Main duties of the job
The post holder will have continuing responsibility for the provision of skilled intervention for people assessed and diagnosed with significant mental health problems in the community.
The post holder will work within a multi-disciplinary service under the supervision of experienced staff. Core responsibilities will include:
* Acting as a Care Co-ordinator/Lead Professional.
* Working within the Care Programme Approach.
* Carrying a varied caseload defined by need.
* Contributing to a duty system on a rota basis.
* Contributing to multi-disciplinary service development.
The post holder will:
* Value and be responsive to the individual rights, needs, and opinions of the people using our services.
* Be responsible for the ongoing assessment, planning, implementation, and evaluation of programmes of nursing care.
* Undertake initial nursing assessments, care planning, and risk assessments as agreed.
* Participate in monitoring the standard of care delivery by Support Worker staff.
* Maintain and promote a patient-centered approach in all aspects of service delivery.

Detailed job description and main responsibilities
Clinical
* Work across a variety of community settings as part of a multi-disciplinary team.
* Provide nursing assessments and identify clients' mental health needs.
* Formulate, implement, and evaluate collaborative care plans.
* Manage own caseload with autonomy until point of discharge.
* Administer depot injections and monitor the effects of prescribed medication.
* Maintain accurate care records according to Professional and Trust standards.
* Act as co-ordinator in accordance with the Care Programme Approach.
* Deliver a range of therapeutic interventions.
* Manage complex mental and physical health crises.
* Escort clients to other Health and Social Care settings when appropriate.
* Offer support and advice to families and carers.
* Identify and manage risks in a variety of settings.
Supervision
* Manage and maintain a caseload with appropriate levels of supervision.
* Provide direct supervision to junior staff.
* Liaise with, advise, and educate other professionals involved in care.
* Act as a mentor for nursing students.
* Receive regular clinical supervision.
* Participate in creating an active learning environment.
* Share identified skills with colleagues and junior staff.
* Identify personal development needs and participate in appraisal.
* Act as a positive role model.
* Offer education and health promotion to clients and carers.

Person specification
Training
* Evidence of current NMC registration.
* Experience of working in a community setting.
* Experience of working with Older Adults in a mental health setting.
Knowledge and Skills
* Ability to respond to the individual rights, needs, and opinions of service users.
* Good risk assessment and risk management skills.
* Knowledge of the mental health needs of older people.
* Excellent written and verbal communication skills.
* Skills in effective care planning and evaluation of individual care plans.
* Knowledge of the MHA/MCA and relevant legislation.
* Basic IT skills.
* Evidence of professional development.
Experience
* Full driving licence and access to a vehicle.
* Relevant post-qualifying experience in mental health nursing.
