Job summary An exciting opportunity has arisen for a Community Mental Health Nurse position within the Mid-Home First Team based in Chelmsford. We are seeking a motivated and enthusiastic individual who is passionate about community mental health. The post holder will join an evolving team, and will be jointly responsible for the delivery of a high standard community care, forming part of a coordinated plan of home treatment to Service Users predominantly within their own homes. Main duties of the job The post holder carries responsibility for the assessment of care needs and their development, implementation and evaluation of programmes of care for Adults and Older Adults with mental health problems that are referred to the Crisis Resolution Home Treatment Team. The post holder is responsible on a day to day basis to the Clinical Lead but ultimately accountable to the Matron for the overall service. In addition, they will be a member of a multi-disciplinary team and work in close co-operation with wards and Primary Health Care based professionals.
