Main duties of the job
THE OPPORTUNITY
Monitor and manage clinical needs of clients in rehabilitation.
Support clients in maintaining physical &personal well-being, including diet, hygiene, and appearance.
Implement, evaluate, and audit care plans and therapy guidelines.
Oversee medication administration and continence care in collaboration with relevant professionals.
Ensure clients dietary needs are met according to dietician guidelines.
Maintain a safe environment, adhere to infection control, and keep accurate clinical records.
Liaise with the multi-disciplinary team and external professionals.
Train and supervise staff, ensuring adherence to Health & Safety standards.
Maintain confidentiality and foster strong team relationships.
Collaborate on effective discharge planning.

Job responsibilities
ROLE RESPONSIBILITIES
· Tomonitor and manage the clinical needs of individual clients who are undergoingrehabilitation at the service.
· Toattend to and assist clients in achieving a high standard of physical/personalwellbeing, including diet, hygiene and appearance.
· Implementand evaluate the efficacy of the care plans and therapy guidelines for allclients.
· Administrationand auditing of medication in conjunction with the Head of Nursing andRehabilitation Support.
· Manage,monitor, assess and implement the continence plans of care for clientsfollowing liaison with the Continence Advisor.
· Monitorthe dietary needs of clients, including guidelines given by the dietician.
· Totake responsibility for maintaining a safe environment and infection
· Controlguidelines.
· Createand maintain legible and accurate records of clinical care.
· Liaisewith the members of the multi-disciplinary team and external professionalsincluding medical personnel as required.
· Toadvise and train staff to the clinical care needs of all clients.
· Tocomply with the requirements of employees under the Health & Safety at WorkAct and to report any incident, accident, hazards to your line manager.
· Observestrict confidentiality in respect of any information regarding clients orinformation relating to the Service.
· Toassist in building and maintaining good staff relationships within thedepartment in order to ensure a high standard of work as an effective team.
· SupervisingSupport Workers whilst on duty.
· Liaisingwith the Head of Nursing and Rehabilitation Support in matters relating to theclients’ clinical needs.
· Inconjunction with the multi-disciplinary team, undertake effective dischargeplanning.

Qualifications
Essential
1. Registered Nurse Adult with valid NMC pin.
2. Evidence of (Prep) post-registration education and practice.
Desirable
3. Mentorship qualification.
