Role summary
In this role, we expect you to lead on the case management of clients with complex needs and work in partnership with other agencies to create holistic, caring, and personalized support plans for clients. This includes identifying and proactively working with our clients towards overcoming barriers, independent living, and sustaining a tenancy. Our clients have a range of support needs that impact their experience of homelessness, including mental health problems, social isolation, challenging behavior, finance and debt, and alcohol and drug abuse. You will work with them to achieve outcomes that include moving on. You will contribute to the delivery of a housing management service, including collecting income and assisting clients with tenancy-related matters.
You’ll also help them identify opportunities for their development, such as moving into employment, and work with them to remove barriers to accessing these opportunities. You will be an advocate for clients, ensuring their voices are heard and making them aware of available support, including maximizing these.
