Job summary The Castleside Day Hospital provides an opportunity for a person-centred approach for older people with a functional or organic illness, There is no strict age range as people are seen depending on their needs and diagnosis The Day Hospital sits within the Community Pathway, under planned care and takes patients with complex needs. The Day Hospital can support people who are in crisis with collaboration from our colleagues in the Community Treatment Team/Crisis team or who are undergoing a relapse of their condition, playing an important role in preventing admission or being an alternative to admission (least restrictive option) The Day Hospital also plays a role in the promotion of early discharge through joint management with our inpatient units and have supported patients to continue treatments such as ECT within the community. The Day Hospital staff are comprised of medics, qualified mental health nurses, nursing assistants and admin and we have a number of other health professionals that work into the unit. We pride our close working relationship with our Community Treatment team/in patient/crisis team/PLT colleagues so we can work in tandem with them if the patients needs determine this, ensuring holistic and timely care. The role of the Band 6 nurse on the unit is to oversee the day to day running of the Day Hospital in collaboration with the Band 7, and help lead the nursing team (please see below) Main duties of the job Function as an independent practitioner, making clinical decisions and providing expert advice. Manage a defined caseload of patients, ensuring comprehensive and timely care for patientswith more complex needs Conduct thorough assessments, develop individualised care plans, and implement interventions and support junior staff to also do so. Administer medication, while monitoring for side effects and adverse reactions. Respond effectively to mental health crises, de-escalate situations, and ensure patient safety whilst providing support to junior nursing and medical staff. Deliver a range of interventions, including psychological interventions and support staff to also do so. Evaluate the effectiveness of care plans and interventions, making adjustments as needed. Work collaboratively with other healthcare professionals, including doctors, psychologists, and social workers. Provide supervision and guidance to junior staff, supporting their professional development. Contribute to the development and functioning of the Day Hospital team. Contribute to the improvement of service delivery and patient outcomes. Facilitate and participate in continuing education programs to enhance the service offered to clients.
