The Clinical Nurse Specialist will provide a comprehensive highly specialist service to patients with a suspected or proven haematological malignancy. The aim of the post is to provide expert care, highly specialist advice and support to both patients with haematological malignancies and their carers, and to patients undergoing investigations prior to diagnosis; this includes patient assessment, and planning and implementing specialist care.
The post holder will act as the key worker for haematology patients from investigation to initial diagnosis and throughout the whole cancer journey to ensure coordination and continuity of care. The post holder will liaise with all members of the multi-disciplinary team, including members of the Primary Health Care Team, to provide specialist nursing knowledge and expertise on all aspects of haematology care.
Clinical Responsibilities:
1. Develop and provide a coordinated specialist service to patients with haematological malignancies and their carers.
2. Have direct clinical involvement in complex care in both the out-patient and in-patient setting.
3. Break bad news of cancer diagnosis and arrange discussion of patients at the multi-disciplinary team meeting in order to facilitate management plan.
4. Liaise with GP within 24 hours of cancer diagnosis.
5. Act as a specialist resource to advise and support healthcare professionals and others involved in the delivery of care to patients, their families and carers.
6. Assess patients and their complex needs and those of their families, and plan, implement and evaluate appropriate programmes of care.
7. Provide emotional, psychological and practical support to the patient and the family/carer throughout the cancer journey and facilitate communication between patients, families and professionals.
8. Ensure that specialist information is available to patients and their families/carers.
9. Act as the key worker/be responsible for nominating the key worker for patients with haematological malignancies.
10. Ensure that clinical practice is effective and evidence-based, with measurable standards and outcomes aiming to improve care and quality of life.
11. Recognise ethical dilemmas relating to care and act as the patient/relative’s advocate when required.
12. Provide an expert clinical service in the delivery of cytotoxic agents by peripheral or central venous access devices, and in the role of checking intrathecal cytotoxic agents.
13. Work proactively in managing change in the speciality to improve practice and health outcomes for patients.
