About the Role:
Are you a compassionate and experienced social worker looking for a role where you can make a significant impact on the lives of children and families? Our client is seeking a dedicated Social Worker to join their Children with Disabilities Team. This role involves providing specialist support to children with disabilities and their families, ensuring that their needs are assessed comprehensively and that they receive the necessary care and services to lead fulfilling lives.
As a Social Worker in this team, you will work with children who have physical, learning, or sensory disabilities, managing complex cases and developing care plans that promote their independence, well-being, and safety. You will work in partnership with families, schools, health services, and other agencies to deliver holistic and person-centred support.
This is a rewarding role that requires a thorough understanding of disability, a commitment to safeguarding, and the ability to work with multi-agency professionals to secure the best outcomes for children.
Key Responsibilities:
* Undertake statutory assessments of children with disabilities, assessing their needs, risks, and protective factors in line with the Children Act (1989) and Care Act (2014).
* Work closely with children and families to develop care plans that address their unique needs, focusing on promoting the child’s independence, development, and well-being.
* Manage a complex caseload of children with disabilities, ensuring timely interventions and support, and keeping accurate case records in compliance with statutory requirements.
* Provide direct support to families, helping them navigate services and providing guidance on how to access benefits, respite care, and other resources.
* Advocate for the child’s needs, ensuring their voice is heard and they are fully involved in decisions regarding their care and support.
* Work closely with education, health services, occupational therapists, and other professionals to coordinate multi-disciplinary support plans for children and ensure their needs are met holistically.
* Conduct safeguarding assessments and ensure children with disabilities are protected from harm, taking appropriate actions where there are safeguarding concerns.
* Attend and contribute to case conferences, multi-agency meetings, and child protection reviews, ensuring that the child’s needs and risks are clearly presented.
* Support the transition of young people with disabilities into adult services, ensuring a smooth handover and continuity of care.
* Provide emotional and practical support to parents and carers, helping them build resilience and access services that support their role in caring for a child with disabilities.
* Ensure all interventions are child-focused, family-centred, and aligned with the principles of personalisation and inclusion.
* Participate in supervision, training, and reflective practice, ensuring continuous professional development and the delivery of high-quality services.
