An opportunity has arisen for an experienced Healthcare Assistant to join the Community Nursing bank with opportunities for shifts in Scarborough, Whitby, Ryedale and Pocklington both in the community and on the inpatient units in Whitby & Malton.
As an HCA working within the community, you need your own transport and to be comfortable working alone in patient homes. This is a 24/7 service with a variety of shift types available including earlies, lates and nights.
Main duties of the job
Day to day the role includes visiting patients to provide the care required according to the care plan written by a registered nurse, participating in handovers and/or safety huddles, and documenting the care provided. Clinical skills used within the role include:
* Vital observations
* Recognise and escalate issues of concern and/or safety including the deteriorating patient and issues of safeguarding
* Wound care
* End of life care
* Moving & Handling of patients
* Equipment provision & instruction in use

Qualifications
* Knowledge of multi-disciplinary working
Knowledge & Experience
* Knowledge of specific role, service & team demonstrated on application
* Experience of caring for someone with a long-term condition or at the end of life
* Experience of working within a community nursing setting
* Experience of using SystmOne
* Able to effectively travel to multiple patient homes and care homes throughout the course of a day including transport of clinical stock required to carry out the treatment
* Sufficient IT skills to learn the systems required for clinical recording, incident reporting and equipment / stock ordering
* Confidence to work out in the community unaccompanied
* Able to work flexible hours to meet service needs including night shifts
* Clinical skills e.g. phlebotomy, bladder & bowel care, wound care, diabetes care, taking vital observations
