Belmont Recruitment are currently looking for an experienced Adults Care Worker to join Worcestershire Council's Adult Social Work Team on an initial 3 month temporary contract with the view to extend. This is a full-time role working 37 hours per week, Monday to Friday.
Key Responsibilities:
* Engage with adults and carers to assess needs and develop creative support plans that promote independence.
* Work collaboratively with multi-agency professionals to ensure holistic care and support.
* Conduct meaningful conversations to determine eligibility under the Care Act (2014) and plan for the future.
* Identify and address safeguarding concerns and support safeguarding procedures.
* Promote access to community resources and build effective relationships with partner agencies.
* Support adults through crises and empower them to achieve positive outcomes.
* Maintain accurate records and participate in reflective practice and professional development.
What We’re Looking For:
* Experience in health or social care, supporting adults with diverse needs, including disabilities, dementia, and long-term conditions.
* Knowledge of the Care Act (2014), Mental Capacity Act (2005), and safeguarding policies.
* Strong communication, assessment, and support planning skills.
* Ability to build productive relationships with agencies and prioritize workloads effectively.
* Level 2 qualification in health and social care (or equivalent).
