OBJECTIVES: To provide a supportive first point of contact for patients, their families and visitors at the practice. To support the clinical teams in their roles and be an active member of the practice team. KEY AREAS OF RESPONSIBILITY The following are the core responsibilities of the Care Navigator. There may be, on occasion, a requirement to carry out other tasks; this will be dependent on factors such as workload and staffing levels: 1. Process and effectively signpost patients to the appropriate healthcare professional depending on the presenting condition 2. Answer incoming phone calls, transferring calls or dealing with the callers requests appropriately 3. Process patient requests for appointments 4. Initiate contact with and respond to requests from patients, team members and external agencies 5. Enter code data onto the clinical system. 6. Photocopy documentation as required 7. Data entry of new and temporary registrations and relevant patient information as required 8. Input data into the patients healthcare records as necessary 9. Direct requests for information, e.g. SAR, insurance/solicitors letters and DVLA forms. 10. Manage all queries as necessary in an efficient manner 11. Carry out system searches as requested 12. Maintain a clean, tidy, effective working area at all times 13. Monitor and maintain the reception area and noticeboards Support all clinical staff with general tasks as request 15. Participate in practice audit as directed by the audit lead 16. Support administrative staff, providing cover during staff absences 17. Scan patient-related documentation and attach scanned documents to patients healthcare records 18. Complete opening and closing procedures in accordance with the duty rota 19. Any other duties commensurate with the role and competencies as deemed appropriate.
