Job Title: Qualified Social Worker (QSW) – Mental Health Team Location: County Hall, Trowbridge, Wiltshire BA14 8JN Pay Rate: £35 per hour Contract Type: Temporary, Full-Time (Hybrid Working) Hours: 37 hours per week Working Model: Hybrid (50% of working hours from home) About the Role: We are looking for a passionate and experienced Qualified Social Worker (QSW) to join our Mental Health Team in Trowbridge, Wiltshire. This is a fantastic opportunity to contribute to a well-established, dedicated social care service that supports adults over 18 with severe and enduring mental health conditions, including dementia. The Mental Health Social Work Teams deliver specialist services to help people recover from mental health challenges and support them to remain as independent as possible. We aim to provide person-centred care that gives individuals choice and control over their lives, empowering them to live with dignity and support in the community. Key Responsibilities: Assessing and Coordinating Care: You will be responsible for conducting assessments of service users and developing, coordinating, and arranging appropriate care plans tailored to their needs. Support for Mental Health and Dementia: Provide support for individuals with severe and enduring mental health conditions, including dementia, both in the community and hospital settings. Person-Centred Approach: Work to ensure that all service users are at the heart of their care, supporting their independence and well-being. Collaborative Working: Engage with local communities and external partners to deliver high-quality, integrated care and ensure the best possible outcomes for service users. Key Requirements: Registration: You must be registered with Social Work England or your relevant professional body. Experience: You should have experience working in a health or social care setting, ideally with adults with mental health needs or dementia. Skills: Strong communication skills, the ability to work collaboratively with other professionals, and a passion for integrated working. Approach: A person-centred, compassionate approach to care, with an understanding of how to engage with local communities and partners to support service users effectively.
