An exciting opportunity to become part of Tameside & Glossop Memory Service and Day Unit for a Band 5 Practitioner. The post holder will work across both services offering assessment and post diagnosis support for adults with a cognitive impairment. The role will also include supporting, assessing and risk management of complex MH needs within a day hospital setting.
The Memory Service provides assessment and diagnosis of suspected memory conditions, initiation, and recommending Cholinesterase Inhibitor treatments for Alzheimer’s Disease as well as post diagnosis support and groups within our Day unit. The position will offer specialist assessments and interventions working in an MDT. There is opportunity to work within the day unit to support individuals during acute mental health crisis and assist the MDT to manage risks.
The post will work closely with senior clinical staff to promote highest standards and values across the unit.
• To perform assessments on receipt of referrals, this maybe in clinic or the
persons home.
• To complete trust approved MH assessments and risk screening for persons referred to Memory clinic or the day unit.
• Complete cognitive and other screening as indicated by presentation and
interpret to inform a MH assessment.
• Complete investigation request forms and liaise with other professional
colleagues involved in the patients care.
• Support the patient and carer through the diagnosis process. Be able to
facilitate carer education and support sessions.
• To ensure any Safeguarding incidents are dealt with and reported according to
Pennine Care policy.
• To contribute to audits as directed by team manager.
• Collaborate with other colleagues and services to develop and embed the offer of post-diagnostic dementia support for service users and carers. E.g., Memory Information groups and the offer of formal Cognitive Stimulation Therapy provided from Whittaker Day Unit.
• Assist and help Maintain the current standards from the accreditation process of The Royal College of Psychiatrists: Memory Services National Accreditation Programme (MSNAP) for the Memory Service on a rolling programme.

Clinical
• To carry a clinical caseload of which are key worked alongside senior staff.
• To demonstrate clinical and practice interventions, supervision and modelling good practice.
• To understand physical health conditions and the impact they can have on
mental health
• To assist in ensuring that the teams are upholding and implementing clinical standards for the care of people with mental health needs within both Memory Service and Whittaker Day Unit.
• Engage and establish an empathic rapport with individuals with a wide spectrum of mental health conditions in both settings
• To undertake the different components of mental health assessments for a range of conditions including robust risk assessments and management of risk following assessment
• To promote appropriate patient engagement and carer involvement.
• To support MDT meetings as required.
• To maintain an up-to-date knowledge of a range of clinical pathways for people with dementia and those with functional mental health issues.
• Be able to effectively support non-mental health colleagues from a range of disciplines.
