We welcome applications for the exciting opportunity to work as a Discharge Tracker within the Transfer of Care Hub at Wirral University Teaching Hospital. This role plays an important part in ensuring that patients have a planned timely discharge and that the patient’s pathway is effectively managed, ensuring patients have a positive experience of the hospital discharge process. The Discharge Tracker works closely with the clinicians and nursing staff to ensure that patients are proactively managed through the system to eliminate any potential delays.
To be first point of call for staff with issues regarding complex discharges. Participate in board rounds and ward huddles.
To work autonomously within identified caseload, managing workload to objectives agreed from daily MDT.
To ensure referrals and notifications are issued in a timely manner.
To develop a rapport with patients and their families/carers, to encourage their interest and participation in planning for leaving hospital.
Identify issues which may affect the patients discharge home and work with the MDT and other agencies to resolve them.
To act as a central point of contact for discharge planning, acting as a resource for knowledge of the discharge planning processes, services available and how to access them.
Identify complex patients at board rounds and ensure all discharge plans actioned.
