Duties and responsibilities There may be queries with some requests, some may be acute. Training will be given to establish who to direct the query to. Add medication from discharge letters and clinical letters to patients electronic record. Refer to the Practice Pharmacist when appropriate To provide first point of contact for patients requesting repeat medication Liaise with Community Pharmacists regarding vulnerable patients The list is not exhaustive. They may also have access to information relating to the practice as a business organisation.
