Clinical Responsibilities To case manage patients with a complex respiratory diagnosis within a community setting without direct supervision, reporting to the respiratory specialist nurse lead, as appropriate, utilising specialist history taking and examination skills. Holistically assess patient need and develop a plan of care that is individualised and meets the needs of patients and carers through physical assessment skills and a respiratory accredited course. Diagnostic spirometry performance and interpretation qualification and registration either through The Association for Respiratory Technology and Physiology (ARTP) or at certified ARTP standard, is desirable but not essential as training will be provided. Possess sound knowledge of supplementary oxygen guidelines, assessment, review and prescribing through blood gas performance and interpretation. Perform all oxygen assessment and reviews as outlined through BTS guidance. Identify, manage and review adjustments in optimising medication within agreed service protocols, through non-medical prescribing. Assist patients to identify and manage risk factors using behavioural change to self-manage their condition. Provide specialist advice to patients/carers in relation to programmes of care which may be complex. Provide sensitive information to patients, where there may be barriers to understand. Decision making/clinical judgment and problem solving. Liase with multi-disciplinary teams, referring as appropriate. To contribute to the development of service specific guidelines and protocols that are evidence based. Work closely with key members of the team from primary and secondary care to alter the management plan as appropriate. Communicate and accurately document any changes in a timely manner.
