A fantastic opportunity has arisen for a Social Worker to join the newly established Transition Team. This new team will support young people in transition, with referrals being made from the age of 14.
As a Social Worker, you will:
1. Receive and manage incoming referrals.
2. Undertake a range of assessments and reviews.
3. Manage safeguarding concerns and enquiries, ensuring a needs-led approach.
4. Focus on community assets in partnership with individuals, carers, and other professionals.
5. Where applicable, arrange appropriate support packages that address the eligible social care needs identified.
Responsibilities will routinely include:
1. Social Care assessments for young people from the age of 16 and carers.
2. Reviews, care and support planning, and continuing health care decision support tools.
3. Adult safeguarding enquiries.
4. Accurately maintaining client records and ensuring they are logged correctly on the department's IT system.
5. Strong report writing and communication skills.
You will also be supported with regular supervision and performance management sessions.
Along with a relevant qualification, you should have relevant knowledge and experience in Adult Social Care, either in a formal or informal capacity, and be able to fully meet the requirements of the post. You must cover the whole geographic area of the City either by possession of a current and valid driving license or by access to public or other transport.
