Main duties of the job

The Social Prescribing Practitioner will co-productively draw up a personalised plan supporting patients to access and engage with the extensive range of support available in the community.

Job responsibilities

Social Prescribing is a short-term intervention service working towards building a diverse and sustainable network of support for those using the service.
The social prescribing approach empowers individuals to take control of their own health and wellbeing. Your role would involve working on a one-to-one basis, offering person-centred appointments/care to support patients' needs, connecting patients to community groups and statutory services for practical and emotional support when appropriate.
Your role is central in developing a relationship with patients; you will be resourceful and creative to find the most appropriate support and build patient relationships. You should have experience of working positively with people facing social and emotional challenges. You will be working with people from diverse cultural and social backgrounds and will need to work confidently and effectively in a diverse and challenging environment.
You will be required to undertake home visits, so you will need to hold a full clean driving licence and access to your own vehicle with insurance coverage for business use.
We're looking for someone who has:
1. Experience of working with people on a one-to-one basis in an information, advice, and guidance health care sector
2. The ability to plan, develop, and manage your own caseload to ensure patient objectives are met
3. To provide person-centred support, agreeing well-being action plans with individual participants, identifying and planning support needs to ensure maximum engagement and improve health, wellbeing, and confidence
4. Knowledge of confidentiality and information sharing protocols
5. Knowledge and practical experience of local safeguarding processes and an ability to operate within relevant policies and procedures to safeguard our patients
You’ll need to be:
1. Well organised and resourceful with the ability to work on your own initiative, meet deadlines, and monitor your own performance
2. Flexible within the practice to attend any meetings in relation to your role, MDTs in relation to patients on your caseload. You will be required to work over four sites within the Immingham, Keelby, and Grimsby area (Meridian Primary Care Network)
3. Confident, outgoing, and have excellent communication skills
4. Able to record accurate information on IT systems
5. Adaptable in ways of working, continuously improving your skills to support our patients effectively
Essential qualifications and experience:
1. Health and social care, coaching, counselling, or other appropriate background relevant to the role
2. Proven experience of planning and goal setting to help improve health and well-being outcomes
3. Proven track record in relationship building skills across a variety of different networks
4. Knowledge of Primary Care and the voluntary and community sector
5. Experience of delivering general and/or specialist advice and information to clients
6. An understanding of Social Prescribing and how it may contribute to health and wellbeing
7. Good IT skills and the ability to complete all necessary client records
8. A confident and professional approach to working with a variety of stakeholders
9. A proven understanding of safeguarding and the ability to implement relevant policies and procedures
10. The ability to work autonomously and to plan, prioritise work under pressure and adapt to new models of working

Communication:
1. To ensure that all communication, both written and verbal, relating to the provision of the service is timely, clear and unambiguous
2. To always communicate with internal and external clients in line with agreed values
3. To prepare for, attend and contribute to Team meetings
4. To engage in regular supervision check-in meetings with the Service Manager
5. To gather and distribute information on available resources in the local community on a continuous basis
6. Promote social prescribing, its role in self-management, addressing health inequalities and the wider determinants of health
7. Build relationships with staff in GP practices, attending relevant MDT meetings, giving information and feedback on social prescribing
