An exciting opportunity has arisen within the Review Team.
The Review Team is a multi-disciplinary team with members from backgrounds that incorporate Social Work, Registered Mental Health Nursing, and Occupational Therapy, who review individuals with a funded package of care. Where appropriate, the team will look at lesser supportive packages to enable increased independence.
Main Duties of the Job
1. Completing reviews of packages of care funded by Manchester City Council and Manchester Integrated Board.
2. Facilitating move on from funded placements where appropriate.
3. Working in a strengths-based way, promoting independence and maximizing resources.
4. Working in accordance with key legislation e.g. Care Act (2014), Mental Health Act (1983), Mental Capacity Act (2005), and Human Rights Act (1998).

Job Responsibilities
Communication
1. Demonstrate high-level communication skills.
2. Utilize highly developed interpersonal skills to facilitate effective communication when there are barriers to understanding.
3. Establish and maintain robust communication networks with service users, carers, and other health and social care workers.
4. Produce reports for court.
Clinical Care, Governance, and Quality
1. Assess complex mental health and social care needs of service users and carers.
2. Produce a support plan for providers which meets the desired outcomes of the individual.
3. Prepare applications to funding panels following reviews.
4. Act as an Approved Mental Health Professional (AMHP) if qualified.
Financial and Other Resources
1. Maximize access to resources across the provider market.
2. Ensure mechanisms are in place to enable Fairer Charging.

1. Participate in regular formal managerial and professional supervision.
2. Engage in continued professional development relevant to the role.
