To work across 7 days on a rotational basis, as part of the ICSS Team to ensure patients in a defined area who have had a stroke have access to specialist assessment and care. There may be a need to provide countywide cover on occasion.

If you are a qualified Physiotherapist who has been out of practice for longer than 2 years, we would consider supporting a Return to Practice programme for this position. The programme would require a mix of private study, formal study and supervised practice, with a requirement to complete a minimum number of supervised hours to update clinical skills and knowledge. Initially the successful applicant would be employed as a Band 4 Generic Rehabilitation Technician for the required timeframe to complete the programme (up to 4 months). On successful completion of the programme and re-registration with HCPC, the post holder would commence employment as a Band 6 Specialist Physiotherapist.
Main duties of the job
* To undertake comprehensive stroke assessment in a community setting, use clinical reasoning and a range of skills to formulate treatment programmes and evaluate care. To work with senior staff to provide this service for patients with complex presentations.
* To interpret and analyse clinical and non-clinical facts to form accurate diagnoses and prognoses within stroke rehabilitation with consideration of co-morbidities and for complex conditions with the guidance and supervision of senior staff. To recommend the best course of intervention, ability to set up, implement and evaluate rehabilitation care plans and potential onward referral.
* To monitor and evaluate treatment to measure progress and ensure the effectiveness of interventions.
* To provide specialist advice, teaching, and training to other members of the MDT, regarding the treatment and management of patients within your speciality and regarding complex home assessments and packages of care. In the absence of senior staff, provide the same advice to Physiotherapy colleagues working within other clinical areas - within the scope of your expertise.
* To provide formal and informal training, supervision and assist in the performance management for more junior staff.
