Primary Care Advanced Nurse Practitioner (ANP)
In recent yearsthe role of General Practice and Community Care has expanded at an exponentialrate. Traditionally GPs with a smallteam were able to meet the vast majority of their patients needs. Over time societal change, population increases,and role expansion has changed the face of General Practice forever. We are seeking to add an additional experiencedNurse Practitioner to join our modern multidisciplinary team. We would like an ANP to work with our SameDay Assessment Team. This job willinvolve triage, clinical assessment, prescribing and provision of same day careto our practice population.
Main duties of the job
Abilityto independently deliver care for the presenting patient from initial historytaking, clinical assessment, diagnosis, treatment and evaluation of care.
Deliver safe, clinical decision-makingand expert care, including assessment and diagnostic skills, for patientswithin the General Practice.
To take on solve andreduce GP workload directly within the scope of their clinical practice.

Job responsibilities
Assess patients presenting with acute illnessor for review of chronic disease conditions.
Skills and ability to consult and triage: FaceTo Face, by Telephone, at Home Visit and by Video Consultation.
To utilize practice IT (SystmOne TPP) forconsultation and all other aspects of clinical care, including recording clearcompetent consultation notes and care plans.
Diagnose, plan, implement and evaluatetreatment/interventions and care for patients presenting with anundifferentiated diagnosis
Clinically examine and assess patient needsfrom a physiological and psychological perspective and plan clinical careaccordingly.
Assess diagnosis, plan, implement and evaluateinterventions/treatments for patients with complex needs.
Proactively identify, diagnose, and managetreatment plans for patients at risk of developing a long-term condition (asappropriate).
Diagnose and manage both acute and chronicconditions, integrating both drug- and non-drug-based treatment methods into amanagement plan.
Fully participate in the clinicaladministration within the practice, signing prescriptions, reviewingmedication, EPS.
Prescribe and review medication for therapeuticeffectiveness, appropriate to patient needs and in accordance withevidence-based practice and national and practice protocols and formularies.
Work with patients and their carers in order tosupport compliance with and adherence to prescribed treatments.
Provide information and advice on prescribed orover-the-counter medication on medication regimens, side-effects andinteractions

Experience
* Some working experience in Advanced Nurse practitioner Role
* Experience and ability to work independently
* Working knowledge of General Practice
* Broad nursing experience and qualification
* Respiratory Special Interest and associated Experience
Qualifications
* Nursing Qualification (degree level or equivalent)
* Advanced Nurse Practitioner Qualification (Msc or equivalent)
* Ability to consult with patients via:
* Face to Face Assessments
* Telephone Triage and Consultation
* Video Triage and Consultation
* Home Visit Consultation and Assessments
* Registration with NMC
* Independent prescriber Qualification or steps towards this
* Further Post Graduate Qualifications or Diplomas
Knowledge and Skills
* Ability to work independently within a Primary Care team.
* A knowledge and understanding of General Practice that empowers the Advanced Nurse Practitioner to understand how to take on and reduce workload from other clinicians.
* To organize and prioritize own work
* Good communication and networking skills
* Ability to problem solve variable situations and variables
* Understanding of the wider primary care health and social aspects
* Knowledge of local services and referral pathways
