Senior Pharmacist, Anticoagulation
Band 8a 22.5hrs 12 months fixed term contract
An exciting opportunity has arisen for an enthusiastic and motivated pharmacist to join the well-respected and progressive Anticoagulation Clinical Pharmacy Team at King's College Hospital.
This post provides an excellent opportunity for a pharmacist to develop their clinical pharmacy and managerial skills in the highly integrated, multidisciplinary thrombosis team at King's College Hospital NHS Foundation Trust.
Main duties of the job
Reporting to the Principal Pharmacist Anticoagulation, your responsibilities will include the following key roles:
1. Deputise for the Principal Pharmacist Anticoagulation
2. Lead and manage clinical pharmacy services in Anticoagulation in conjunction with the Principal Pharmacist
3. Participate in multidisciplinary ward rounds delivering and promoting high quality, cost-effective, patient-focused pharmaceutical care
4. Consult with primary care colleagues to ensure the safe initiation and management of anticoagulation
5. Act as a specialist role model and an expert in the prevention and treatment of thrombosis and the management of anticoagulated patients
6. Supervise and train rotational pharmacy staff
7. Ensure a high level of clinical pharmacy input to out-patients in the anticoagulation clinics and in-patients on the multidisciplinary ward round
8. Promote safe drug use for anticoagulated patients across all specialities
9. Develop, implement and audit local guidelines
10. Education and training of healthcare professionals within and outside the Trust relating to the safe use of anticoagulants
11. Patient/carer education as part of discharge planning and close liaison across the interface
12. Participate in practice and clinical research and audit
13. Work with the Principal Pharmacist Anticoagulation to extend the boundaries of professional practice within the anticoagulation team
Person specification
Qualifications
Essential criteria
* MPharm (Masters Degree)
* Registered Pharmacist (1 year competency assessed and examined professional registration with the General Pharmaceutical Council)
* Mandatory CPD to maintain fitness to practice
* Postgraduate Diploma in Pharmacy Practice or equivalent qualification / experience
* Registered Independent Prescriber or willingness to complete

* Member of the RPS faculty
Experience
Essential criteria
* Specialist clinical knowledge and skills relevant to area of practice
* Demonstrates awareness of and commitment to the clinical governance agenda
* Understands national and local priorities
* Knowledge of regulations and legislation relevant to area of practice
* Experience of working with senior healthcare professionals outside of Pharmacy
* Experience in directorate pharmacy
* Experience of practice research or audit
* Experience of protocol/guideline development
* Experience of multi-disciplinary team working
Desirable criteria
* Demonstration of advanced practice using evidence mapped to the RPS Advanced Practice Framework
Skills
Essential criteria
* Be able to demonstrate advanced level of clinical reasoning and judgment
* Be able to communicate effectively in both written and verbal formats with staff and patients
* Be able to provide highly complex Information on medicines use, clinical pharmacy and financial issues to all levels of healthcare professionals (up to and including senior consultants/professor level)
* Be able to work at any King's College Hospital NHS Foundation site as required
* Able to participate in Pharmacy seven day service including the Late Duty, Weekend and Bank Holiday Service
* To have basic keyboard and computer skills (word processing, spreadsheet, report generation and presentations)
