Job overview
An exciting opportunity has arisen for 2 registered nurses to join our forward-thinking and Gold Award-winning Community Team in Nantwich and Rural. These are both 12-month temporary vacancies covering maternity leave. The hours are 37.5 hours (5 x 7.5 hours a week) covering a 7-day service.
This post requires effective communication and interpersonal skills and the ability to be adaptable, reliable, and approachable. The successful applicant will have a sound knowledge of recent changes within the NHS and knowledge of the political drivers to provide care closer to home.
As part of a team, you will be responsible for the assessment of nursing care needs, implementation and evaluation of care programmes, and the delivery of high-quality holistic care within the community setting and within our wound care clinics. You will be expected to support your peers, the team leader, and the organization in promoting the Trust's values and behaviours.
You will need to be able to work independently, possess good time management and organizational skills, and be expected to be innovative and flexible to work in other areas if required to meet service needs.
Travel is an essential part of this role; you will need a full UK Driving license and use of a car. Please demonstrate on your application form how you would meet this requirement.
Main duties of the job
We are looking for 2 Band 5 staff nurses to cover maternity leave to support the patients in the Nantwich Care Community Team. The integrated team consists of Complex Case Practitioners, District Nurse/Case Managers, Macmillan, Therapy teams, and Administration Team delivering high-quality, efficient, and safe care.
We are looking for enthusiastic, caring, and compassionate nurses who possess a good clinical skill set. They will also be able to holistically assess patients' needs whilst ensuring health promotion and quality improvement to meet the needs of our patients. You must have a good knowledge of modern IT software applications.

Detailed job description and main responsibilities
1. To work under the direction of the Community Nurse Team leader and be responsible for the assessment and management of the health needs of an identified population and for the development, implementation, and evaluation of programmes of care within a range of target groups including end of life care and patients with long-term conditions and complex nursing needs.
2. To support, and wherever possible maintain patients in their own preferred place of care, enabling them to maximize their independence and optimize their quality of life.
3. To liaise with all healthcare professionals and other agencies to ensure appropriate and effective service delivery.
4. To work within the agreed framework for Community Nursing Services.
5. To assess, provide, and review holistic nursing care, advice, and information to patients and carers within the home environment or appropriate care setting in a safe and professional manner using evidence-based knowledge.
6. To undertake initial assessments on receipt of specific referrals to the caseload, acknowledging the needs of clients and their carers.
7. To assess, plan, implement, and evaluate a plan of care to meet the patient’s physical, social, emotional, and environmental needs, encompassing a rehabilitative/self-care approach where necessary and as per district nursing contract.
8. To provide skilled nursing care and the ability to undertake clinical tasks including wound care, leg ulcer assessment and management, administration of medication via syringe pump, care/use of intravenous lines, venepuncture, and catheter care.
9. To be able to recognize, interpret, and analyze clinical data related to the individual client's needs including client observations.
10. To demonstrate, teach and supervise the nursing care that can be safely delegated to the patient/carer giving support as necessary.
11. To promote clinical excellence and an evidence-based approach to care by complying with the agreed Trust policies and procedures and maintaining clinical competencies in line with the competency framework.
12. Work to the End of Life (EOL) Strategy including adherence to all EOL tools, acting as a clinical expert in EOL care provision.
13. Take an active role in health education of the individual and community promoting self-care and providing both planned and opportunistic care based upon evidence-based information.
14. Accurately record all patient contacts on Emis web and Trust documentation as per MCHFT policy ensuring patient and professional confidentiality is maintained at all times in accordance with NMC guidelines.
15. Manage and assess patients' discharge potential and need from the caseload.
16. Ensure that equipment and aids necessary for effective care meet the needs of individual patients and are adequately maintained. Demonstrate and explain the safe use of equipment for patient and/or carer where required.
17. Participate in the delivery of care involving complex needs and supporting carers and relatives in distressing circumstances.
18. Refer people to other practitioners when needs and risks are beyond one’s own scope of practice or require longer-term support.
19. Prioritize and manage own caseload & client group.
20. Assume the responsibility for the clinical caseload in the absence of the Team Leader/Senior Lead delegating work appropriately within the competency framework.
21. To participate in the measurement and monitoring of quality standards as appropriate.
22. If a qualified nurse prescriber, to participate fully in the Non-medical Prescribing initiative ensuring assessment of prescribing need is evidence-based and cost-effective and adheres to MCHFT wound care prescribing formulary.
23. To maintain accurate personal and professional records adhering to MCHFT and Nursing and Midwifery Council policy and guidelines.
24. To ensure the most efficient and cost-effective utilization of all resources (human, physical, and financial).
25. To report accidents/incidents of patients or self which occur whilst on duty and to complete on the Incident reporting system immediately. To participate in any investigations as necessary.
26. To contribute to and cooperate in the coordination of off duty and holiday leave with colleagues, ensuring service cover is maintained. To work effectively to support other team members in all aspects of the service.
27. To maintain all aspects of MCHFT Integrated Governance Strategy and requirements including active participation in appraisal and clinical supervision in accordance with Trust policy.
28. Act at all times in a manner that illustrates respect for privacy, dignity, and confidentiality.
