Implement rehabilitation and therapy programmes in collaboration with healthcare professionals. Monitor and document progress, reporting any changes to the care team. Support clients in accessing social, recreational, and vocational activities to promote independence. Communicate effectively with families, carers, and medical professionals to ensure continuity of care. Maintain a safe, respectful, and nurturing environment for clients.
