The SACT Team is keen to recruit an enthusiastic and motivated permanent band 6 nurse. You will be joining a newly revised and evolving service that supports 6-day working and provides primary assessment and support for patients receiving oral SACT. This opportunity will support you in developing skills in becoming a UKONs telephone triage nurse, where you will participate in the rota to triage patients over the phone and direct them onto appropriate pathways. Opportunities for continued learning and involvement in the UKONs forum are available.

Main duties of the job
* The post holder will work as an important member of the Multidisciplinary Team within the Torbay and South Devon SACT service.
* Collaborate with a range of health care providers in the local community and across the regional Network.
* Provide expert advice and management for SACT patients.
* Actively promote self-care for all patients in the Torbay and South Devon Health Authority, including hospice patients.
* Utilize skills of clinical expertise, education, and management to ensure a seamless service for patients, carers, and staff, providing advice, support, and information from the time of diagnosis and throughout curative, palliative, and end-of-life care.
* Work as a supportive and inspirational team member within the integrated teams, providing a high standard of profession-specific expertise.
* Actively engage in evidence-based practice.

* Demonstrate at an advanced level, communication skills that provide appropriate support, relevant information, and education to facilitate decision-making on all treatment options for patients and their family/carers.
* Promote and ensure effective communication within the multidisciplinary team, developing close partnerships and productive working relationships with key individuals and groups. Communicate and liaise with GPs/hospital consultants and other healthcare professionals as appropriate regarding patient issues.
