Livewell Southwest has an exciting opportunity within the Community Neuro Rehab Team (CNRT).
The team are looking to recruit Band 3 Therapy Support Workers that will work as part of the multidisciplinary team.
The CNRT based at Mount Gould Hospital, Plymouth has recently gained financial investment to realign and reshape the support offered to patients in Plymouth, South Hams and West Devon to meet the National service model for an Integrated Community Stroke Service (ICSS).
The Therapy Support Workers work within the MDT to provide outpatient and community based multidisciplinary specialist stroke and neurological rehabilitation.
The team are currently hybrid workers with opportunity to work from home and office when not treating patients in the community.

Main duties of the job
The role includes:
1. To support all aspects of therapy assessment and treatment of patients within the role setting. Facilitate a wide variety of therapeutic interventions and activities depending on service needs (this may include, one to one, group or joint working with colleagues).
2. Provide support and supervision to patients to enable access and participation in community-based support and social inclusion services.
3. To facilitate individual activities for specified patients working within risk management guidelines. This may include working alone without direct supervision for periods of time under the guidance of a registered professional/Manager.
4. To participate in the planning and development of the service and for the Directorate as a whole. This may involve participating, where appropriate, in defined projects.
5. To take a holistic approach to client management, carry out tasks and treatment plans as directed by senior staff. To feed back to the appropriate worker and maintain accurate clinical records.

Job responsibilities
Clinical:
1. To provide a continuing element of the overall therapy programme/treatment plan to clients within the setting. This may include clients with additional physical health needs, physical disability, behaviours that challenge and learning disabilities.
2. The post holder will be involved with monitoring the patients' progress, making some treatment decisions while assessing and re-assessing the patients' progress.
3. The post holder will observe general behaviour, functional ability, and response of patients and record the information liaising with the senior therapist/worker.
4. To undertake client interventions and reviews, regularly meeting the relevant professionals to discuss progress, and reporting back if any problems occur.
5. To seek advice and supervision from the senior therapist/worker within the team as necessary to ensure appropriate patient management and care.
6. To use knowledge and skills to engage patients in therapeutic activities to promote healthy psychological and physiological well-being. Encouraging self-management techniques where able.
7. To prescribe basic equipment and minor adaptations under prescribers' guidelines and with guidance, where required, from a senior therapist.
8. Deliver, collect and instruct in the use of equipment for patients as part of their treatment plan.
9. Report defects in equipment, taking action to ensure such equipment is withdrawn from service.
10. Be responsible for material stock and maintenance stock levels as required. Order equipment from the Community Equipment Service.
11. Where applicable, to participate in reviews/case conferences of individual patients and contribute to the evaluation of their progress and development of their goals.
12. To attend multi-disciplinary/multi-agency case conferences, liaison, meetings, handovers and patient reviews as necessary and appropriate.
13. To support and encourage patients, their relatives and carers in an active approach to regaining and maintaining maximal functional independence.
14. To be responsible for contributing to the planning, monitoring and evaluating of patient centred individual and/or group activities as part of a regular review of the service requirements if required within your team.
Responsibility for People Management:
1. Participate in induction and support programmes in relation to new staff and volunteers where appropriate.
2. To receive line management support and supervision.
Responsibility for financial and/or physical resources:
1. The post holder will have no direct responsibility for finances, however they will adhere to the relevant trust policies on financial issues.
2. The post holder will use Livewell Southwest resources responsibly.
3. The post holder may be required to record and manage patients' property as part of their role.
Responsibility for administration:
1. Prioritises workload effectively, this could include scheduling your workday appropriately or managing your own diary of appointments and visits (as well as mileage if applicable). This requires a high level of time management and organisation skills.
2. To be aware of, and adhere to, the correct procedures for ordering, maintaining and using materials and equipment as directed by registered staff.
3. To participate in the planning and implementation of operational policies, projects and service development within the team.
4. To maintain accurate, comprehensive and up-to-date client records and related documentation in line with clinical guidance and LSW policy.
5. To be aware of, and act upon, LWSW and safety policies, including the prompt recording of any accident, incident, or other untoward event involving self, patients or other staff.
6. To keep abreast of policy changes and developments within LWSW and implement relevant LWS policies, protocols and guidelines.
7. To take a flexible approach to supporting colleagues in times of caseload pressure, including working with colleagues to maintain support across the team within the resources available.
8. Report any changes in the patients' condition or care needs to the relevant therapist and document accordingly.
9. Maintain confidentiality of information being aware of trust policies and local protocols and keeping up to date with changes.
10. To access email and other information through Livewell Southwest intranet.

Qualifications
* Regulated Qualifications Framework (RQF) level 3 qualifications (e.g. A levels or T-Levels) or demonstrated ability and commitment to achieve a relevant level 3 qualification.
* Level 2 functional skills, or as a level 3 apprenticeship such as the Senior Healthcare Support Worker apprenticeship (or other level 3 qualification pathway).
Experience
* Experience of formulating therapeutic relationships.
* Previous experience of working within a healthcare setting or similar role (employment, work experience, volunteering).
