We are looking for anenthusiastic, dedicated, and motivated Nurse who wouldlike the opportunity to work within our Older Adult Community Mental Health Teamwith patients that have functional and organic mental health needs.
The core hours for our team isMonday to Friday (9pm -5pm) and this specific post is for hours per week.
Youwill be part of an integrated team consisting of Community Mental HealthNurses, Occupational Therapists, Consultant Psychiatrists, Junior Medics, aClinical Psychologist, Support Workers, a Trainee Advanced ClinicalPractitioner and Administration staff. You will also be supported by the TeamManager who offers a robust supervision and appraisal process.

Our OAMH service has developed a range of therapeuticinterventions which has a Solution Focused approach at the core of its offer.

Main duties of the job
You will manage a caseload of patients with functional and organicmental health conditions from assessment to discharge and engage in therapeuticinterventions with them.
Youwill develop the skills to undertake full Psychosocial assessments, individualcare plans and risk assessments with patients and their carers.
Attend MDT meetings on a weekly basis todiscuss allocations, complex cases and there is also a lot of opportunity forjoint working.
Be involved in our Well-being clinic, which iscomprised of depot administration, medication reviews and post-diagnosticsupport.
Being involved in Solution focused supervisionsessions, Reflective practice, and additional training opportunities.
To be provided with the opportunity to becomeinvolved in CST (Cognitive Stimulation Therapy) groups, Carers groups and toformulate a group within the Mental Health Needs Pathway.
The Older Adult Mental Health service is comprised of severalspecialist teams, and you will have the opportunity to work with our Inpatientwards and Hospital at Home Team to meet the needs of more acutely unwellpatients.
