Within our busy Neighbourhood Team, we arelooking for a Community Nurse to work between 22.5 and 37.5 hours per week fora period of 12 months. Passion aboutnursing, excited by change and keen to help shape a new way of deliveringnursing care in the community is essential.We are a multi-disciplinary team that work closely with our GPs andwork hard at developing relationships with other primary care services, drawingon skills to manage complex patients and ensure our services are as responsiveand patient centred as possible.

Main duties of the job
The role requires

Your typical working day willinclude home visits for adults over 18 years needing a range of nursinginterventions, including end of life care, all types of wound care- you willbecome expert in tissue viability. Other skills required will be in IV therapy andholistic assessment of a persons needs, taking in to account their environmentand social circumstances.
Youwill support in the education of student nurses pre and post registration, plusnurse associates and HCAs to a high standard. As a band 5 nurse, you willoversee patients and assist in decision making regarding their care. You willalso attend daily handovers and participate in meetings.

Job responsibilities
The post holder will:

This will be achieved through acombination of hands on clinical skills and knowledge to treat, educate andsupport patients on a designated caseload and by working with patients andtheir carers to assess, plan and implement appropriate packages of care.

Qualifications
* Registered Nurse (RN1)

* Accredited courses in palliative care, tissue viability, Diabetes
Experience
* Experience gained during Pre-Registration training
* 2 years post registration experience
* Community nursing experience
* Experience of management of caseloads
Knowledge
* Variety of evidence based clinical skills
* Confidence to teach student nurses, trainee nursing associates, patients, carers and members of the multidisciplinary team
* Understanding of evidence-based nursing, multidisciplinary working and clinical governance
* Competence in male and suprapubic catheterisation
* Venepuncture and IV therapy
* Cannulation
* Management of syringe drivers
