Job summary An exciting opportunity has arisen within Stockport NHS Foundation Trust's Hepatology Department. We are looking for a motivated, enthusiastic individual with administrative experience to join our Hepatology team. By becoming a Hepatology Pathway Navigator you will be part of a community of similar roles across Greater Manchester, with Pathway Navigators being based in most, if not all, Trusts in the region. Working closely with the Gastroenterology Management and Clinical Teams and Trust Cancer Navigators you will have a key role in ensuring that patients have a cancer diagnosis as early as possible, supporting those patients in the early stages of their pathway by being their first point of contact up until diagnosis. You will be vital to ensuring new Straight to Test (STT) pathways achieve their targets, whilst maintaining high patient experience outcomes. You will work with patients, their carers and health care professionals to assist in co-ordinating the appointments and information required by clinicians in the pathway to deliver the faster diagnosis standard. Main duties of the job Co-ordinating the rapid booking for patients requiring urgent CT or MR imaging due to suspicious liver lesions, as well as other urgent tests such as blood tests. Co-ordinating the booking of timely ultrasound scans for patients due their 6 monthly routine HCC surveillance imaging. Communicate with patients to ensure that they are well prepared for their clinical investigations. Providing advice and information regarding all aspects of investigations. Be the first point of contact for patients, their significant others and Health Care Professionals, up to the point of diagnosis, working closely with the relevant Nursing Teams. Contribution to the achievement of patient access targets through continual validation of patient pathways, and patient waiting lists escalating any identified issues or delays to the attention of clinical and or management teams. Liaise with all members of the MDT to aid facilitation of a collaborative approach to the care of the patient and contribute to the MDT meetings as and when required. Actioning outcomes following the MDT meeting, including the coordination of sending patient details to tertiary centres for specialist opinions or treatment. Continued in Job Description.
