We are looking for a Community Care Officers - to join the Early Intervention Vehicle within Hertfordshire County Council. The Role The successful candidate would join an established integrated Health and Social Care Team on the Early Intervention Vehicles The service is currently based from health care offices in Watford As a social care professional they will be working alongside nurses, paramedics, physiotherapists and occupational therapists There are two Early Intervention Vehicles which cover South West Hertfordshire The service operates 8am - 6.30pm 7 days a week - some weekend work is required The vehicles provide immediate health and social care assessments. Preventing individuals at risk of hospital admission to remain at home Candidates must have full driving licence Previous CCO experience Understanding of legislation Fast paced work Experience preferred: Minimum of 12 months' experience of working within the Health and Social Care sector as a Support Worker in the UK The team: Well-structured organised team, growing their staffing levels Minimum Requirements: 1 year experience in a similar role within the UK Enhanced DBS/CRB (we can obtain for you) Eligibility to work in the UK NVQ level 2 in Health and Social Care To discuss this role, or any similar vacancies we have available in Hertfordshire, please contact Faye Whitehead on (phone number removed) or email to
