This is a critical Occupational Therapy / Physiotherapy role in delivering rehabilitation to patients residing in the Non-Weight Bearing (NWB) Pathway beds in Hawkinge House Proactive Assessment Unit, Folkestone, East Kent. You will be responsible for supporting recovery and rehabilitation, and working alongside the MDT to assess and plan discharges for patients placed on the NWB pathway.
Main duties of the job
The post holder will work within the Non Weight Bearing (NWB) Pathway supporting discharges from acute hospitals in East Kent. These beds are situated within Hawkinge House Proactive Assessment Unit, Folkestone, managed by the Community Assessment Bed team, where the post holder will be working as part of a Multi-Disciplinary Team providing therapy interventions to support Recovery, Rehabilitation, Assessment and Discharge Planning for patients recovering from fractures. The pathway incorporates 6 weeks of recovery and healing, during which time therapy is provided to support recovery from non-weight to partial and full weight bearing, and then working with patients to achieve rehabilitation goals to support discharges home.
You will also provide evidence of delays in transfers of care and raise appropriate escalations when patient flow may become compromised. This is currently a 5 days a week service - core hours are Monday to Friday 8am-5pm (37.5 hours a week).

Person Specification
QUALIFICATIONS
* Registered health care professional (HCPC registration) / minimum of 3 years of experience in health improvement environment. Health related degree and evidence of relevant post graduate development at Level 6. Evidence of Specialist qualifications relevant to area of practice.
* PRINCE 2 or equivalent project management training. PTLLS/DTLLS, PGCE or Certificate in Education Management or supervisory qualification or experience of study. Working towards Master level study.
EXPERIENCE
* Minimum of 2 years' experience in area of practice. Practical experience in managing health related projects/complex caseloads. Supporting others (mentor/educator/supervisor) experience.
* Experience of leading and managing teams. Experience with the use of Microsoft Office and a variety of IT packages (E-Pay/E-roster/AT-P/CIS).
SPECIAL KNOWLEDGE AND EXPERTISE
* Understanding of National Service Frameworks and NICE guidance relevant to care setting and its application across health care services. Evidence of continued relevant professional development and implementation in practice within the last 12 months.
SKILLS & ABILITIES
* Understanding of training needs in relation to post. Ability to manage time effectively, prioritize and work under pressure to meet deadlines. Ability to work effectively with a variety of people at all organisational levels across different sectors including areas of social deprivation. Ability to communicate complex, sensitive information to individuals and groups. Strong influencing and negotiating skills.
