The Community Mental Health Team is seeking a skilled, motivated, organised, and enthusiastic Mental Health Nurse to work within an established, integrated, multi-disciplinary team. The focus will be on individuals with severe and enduring mental health problems. The successful post holder will be responsible for care co-ordination and case management of a defined caseload of clients with complex and challenging needs, as specified within Parts of The Mental Health (Wales) Measure.
It is desirable for candidates to speak Welsh; applications from English and/or Welsh speakers are equally welcome.
Main duties of the job
1. To be responsible for the care co-ordination of a designated caseload of clients with severe and enduring mental health problems.
2. To ensure adherence to Care and Treatment Plans and meet standards in line with the Mental Health (Wales) Measure.
3. To act as a point of contact for GPs and other interface services, offering advice, support, and signposting to appropriate agencies.
4. To formulate and develop standard and complex outcome-focused care and treatment plans.
5. To ensure comprehensive risk assessments are completed and documented for actual and potential risks.
6. To support clinical duties such as assisting with depot/clozapine clinics and supervising the administration of long-acting injections.
7. To complete physical health assessments and monitor side effects for patients prescribed Clozapine.
8. To assess individuals in crisis, perform comprehensive assessments of health and social care needs, and agree on the most appropriate course of action.
9. To assess, evaluate, and manage clinical risk, developing care plans and delivering evidence-based crisis interventions.
10. To work collaboratively with service users, carers, and partner agencies to deliver high-quality care.
