What you will do:
1. Conduct home-based initial assessments to evaluate patient needs and develop tailored care plans.
2. Create home exercise programmes and order or review necessary equipment provisions to support patient recovery.
3. Collaborate with and allocate tasks to rehabilitation support workers to ensure holistic care delivery.
4. Review patient progress regularly to adjust care plans and optimize outcomes.
5. Although it's preferable, being a car driver is not essential for this role.
Requirements:
1. Hold a valid HCPC registration.
2. A minimum of 2 years of relevant experience within the physiotherapy field.
3. Car driver preferable but not essential.
