Job summary North Hardwick & Bolsover Primary Care Network (PCN) is seeking to recruit a social prescribing link worker to join our existing PCN team. We are small, dynamic PCN representing approximately 50,000 patients across 7 GP practices. The candidate would be expected to work with patients across the practices and in the local community. It is essential that you have a car to travel within the PCN footprint as part of this role.
