Bay Medical Group are looking to recruit a number of Pharmacists for a fixed 10 month period to cover maternity leave. Full and part time hours are available.
Interviews scheduled for week commencing 16th December 2024.
Main duties of the job
Collaborating with a team of pharmacists, technicians, and other members of the medicines management team (MMT).
Work as an integral part of a multi-disciplinary team, contributing both administratively and in direct patient-facing interactions.
Conducting clinical medication reviews to proactively manage patients with complex polypharmacy, ensuring their treatment regimens are both safe and effective.
Serve as a primary point of contact for general practice staff, providing expert advice on prescription and medication queries. They will play a crucial role in supporting the repeat prescription system, handling acute prescription requests, conducting medicines reconciliation during transfers of care, and implementing systems for safer prescribing. Additionally, they will offer expert clinical guidance on medicines, addressing the public and social care needs of patients.
Take a leading role in medicines optimisation and quality improvement, overseeing aspects of the quality and outcomes framework as well as enhanced services.
Ensuring seamless integration between community and hospital pharmacies, the post holder will contribute to the efficient use of skill mix, enhancing patient outcomes, improving healthcare access, and helping to manage workload. This role is vital in driving improvements in both the quality of care and operational efficiencies, requiring a high level of motivation and a passion for delivering exceptional service within general practice.

Primaryduties and areas of responsibility
Clinics/Clinicalassessment (Tasks/AccuRx)
Conductboth face-to-face and telephone consultations with patients diagnosed with oneor more medical conditions, where medicinal intervention is necessary (,COPD, asthma).
Forpost holders who are independent prescribers, prescribe safe, effective, andappropriate medications in accordance with current clinical guidelines andrelevant legislation.
Clinical medication review
Conductclinical medication reviews with patients, providing recommendations to seniorclinical pharmacists, nurses, and/or GPs regarding prescribing and monitoringdecisions.
Assessthe ongoing necessity of each medication, review monitoring requirements, anduse each consultation as an opportunity to support patients in managing theirmedication effectively, ensuring optimal use of their medicines throughmedicines optimisation and structured medication reviews.
Care homemedication reviews
Conduct clinical medication reviews forpatients, collaborating with care home staff and community pharmacists toenhance the safety of medication ordering and administration processes.
Domiciliaryclinical medication review
Undertakeclinical medication reviews with patients, providing recommendations to seniorclinical pharmacists, nurses, and GPs regarding prescribing and monitoring.
Attendmultidisciplinary case conferences and refer patients as needed.
Managementof common/minor/self-limiting ailments
Managea caseload of patients with common, minor, or self-limiting conditions, workingwithin the scope of practice and limits of competence.
Signpostpatients to community pharmacies and refer to GPs or other healthcareprofessionals when necessary.
Medicinessupport
Offerpatient-facing or telephone clinics to address questions, concerns, and queriesregarding medicines.
Respondto medicine-related inquiries from GPs, practice staff, other healthcare teams(, community pharmacists), and patients.
Suggestappropriate solutions and follow up with patients to assess the impact of anymedication changes.
Unplannedhospital admissions
Review the use of medicines frequentlyassociated with unplanned hospital admissions and readmissions through auditsand individual patient reviews.
Implement strategies to reduce theprescribing of high-risk medications in vulnerable patient groups.
Managementof medicines at discharge from hospital
Workwith the Medicines Management Team (MMT) to reconcile medications followingdischarge from hospitals, intermediate care, and care homes.
Identifyand rectify unexplained medication changes, collaborating with patients andcommunity pharmacists to ensure continuity of care post-discharge.
Establishsystems to maintain a consistent supply of medications for high-risk patients(, those using compliance aids or in care homes).
Signposting
Ensure that patients are referred to theappropriate healthcare professional for timely care, including pathologyresults, common/minor ailments, acute conditions, and long-term conditionreviews.
Repeatprescribing
Oversee the practice's repeat prescribingpolicy and reauthorization process.
Ensure that patients undergo necessarymonitoring tests when required.
Risk stratification
Identify patient cohorts at high risk of harmfrom medicines through pre-prepared practice computer searches, consideringboth patient- and medication-related risks.
Servicedevelopment
Contribute pharmaceutical advice for the developmentand implementation of new services that have medicinal components ( adviceon treatment pathways and patient information leaflets).
Informationmanagement
Analyse, interpret and present medicines datato highlight issues and risks to support decision making.
Medicinesquality improvement
Conduct clinical audits of prescribingpractices, providing feedback and implementing improvements in collaborationwith the practice team.
Medicinessafety
Implement changes in response to MHRA alerts,product withdrawals, and other local or national guidance to enhance medicinessafety.
Implementationof local and national guidelines and formulary recommendations
Monitor practice prescribing against thelocal health economys RAG list and make recommendations to GPs for medicinesthat should be prescribed by hospital clinicians (red drugs) or subject toshared care (amber drugs).
Assist the practices in maintaining thepractice formulary that is hosted on the practices computer system.
Audit the Practices compliance against NICEtechnology assessment guidance.
Provide newsletters or bulletins on importantprescribing messages.
Educationand training
Provide education and training to the primaryhealthcare team on therapeutics and medicines optimisation.
CareQuality Commission
Collaborate with the general practice team toensure compliance with CQC standards related to medicines management.
Publichealth
Support public health campaigns and provideexpert knowledge on available public health programmes.
Collaborativeworking relationships

Knowledge,skills and experience required
Hold an undergraduate degree in pharmacy andbe registered with the General Pharmaceutical Council.
Possess experience and awareness of commonacute and long-term conditions typically encountered in general practice
Recognise priorities when problem-solving andidentifies deviations from the norm, referring to senior professionals whennecessary.
Adhere to legal, ethical, professional, andorganizational policies, involving patients in decisions about theirmedications and supporting adherence in line with NICE guidelines.

15. Experience in working with computerised health care systems.

18. Registrant of General Pharmaceutical Council (GPhC) and commitment to CPD.
Desirable

20. Hold post graduate clinical Diploma or equivalent.

22. Completion of either the Primary care pharmacy education pathway, or MSc Advanced Practice Pathway.
